Online Spring Student Evaluation Instructions

  • Evaluations missing any required signatures, including the student’s, will be returned. 
  • Annual Evaluations are required for ALL students except for students currently on internship.

NOTE: If you are unable to connect to the UC San Diego online evaluation, it is most likely because you are not on a Virtual Private Network (VPN) from your home/coffee shop. If you experience problems logging into the Grad Division website, you will need to first VPN into campus. For directions, visit the “Virtual Private Network (VPN) at UCSD” webpage. 

For Students: 

  1. Schedule a formal meeting with your Guidance Committee to assess your progress before the end of UCSD Spring Quarter
  2. Prior to your Committee meeting, you should prepare a summary of your progress in the past year to distribute to the Committee at the beginning of the meeting.
    • At minimum that summary should consist of:
      • an updated CV that includes any publications, presentations, awards, or grants obtained in the past year
      • for students still completing coursework, a summary of all courses taken, including the grade received in each
      • future courses and other milestones yet to be completed and the timeline and anticipated date of completion for each of the remaining degree requirements
  3. Once the online evaluation has been completed and signed by appropriate Guidance Committee Members, you will receive an automated email through your UCSD email address instructing you to login and sign your evaluation.
    • PLEASE NOTE: Make sure to check your UCSD email address OR update your account so that your emails are forwarded to your preferred email.
  4. Login to UCSD’s online evaluation site at https://gradforms.ucsd.edu/portal/student/index.php
  5. Click on the “Spring Evaluation” button and log in using your Single Sign-On account
  6. Review the completed evaluation
  7. Sign and Submit (You are required to sign your Spring Evaluation)

Note: You will be able to view or download a pdf of your evaluation at any time. 

 

For Student Mentors/Committee Chairs:

Mentors/Guidance Committee Chairs are required to complete all fields and sign the online Spring Evaluation for your student(s).

  1. Login to UCSD’s online evaluation site at http://grad.ucsd.edu/resources/staff.html
  2. Click on Online Portals >> Faculty Portal for Spring Evaluations URL and log in using your Single Sign-On account
    • SDSU faculty can access this site using your affiliate account. Please contact the UCSD Program Coordinator, Kristin Deveraux (kdeveraux@ucsd.edu) if you need help accessing the site.
  3. Click on the appropriate box titled “evaluation waiting your signature and comment”
  4. Click on the student being evaluated and complete the evaluation accordingly
    • For more information about an IDP, you can find it on our website under our Individual Development Plan (IDP) webpage.
    • The department questions for faculty are required
    • Remember, paper copies of the evaluation will no longer be accepted!
  5. Sign and Submit

Note: You will be able to view the evaluation(s) for your student(s) at any time.

 

For Guidance Committee Members:

Guidance Committee Members are required to sign the online Spring Evaluation for your advanced to candidacy student(s). You may log in, make comments and sign at your leisure for non-advanced students. 

  1. Once the evaluation has been completed and signed by the Mentor/Guidance Committee Chair, then viewed by our staff, you will receive an automated email instructing you to login and sign the evaluation. 
    • PLEASE NOTE: You can view the evaluation(s) for your student(s) at any time but will only be able to sign after the evaluation has been submitted by the Mentor/Committee Chair.
  2. Login to UCSD’s online evaluation site at http://grad.ucsd.edu/resources/staff.html
  3. Click on Online Portals >> Faculty Portal for Spring Evaluations URL and log in using your Single Sign-On account

  4. Click on the appropriate box titled “evaluation waiting your signature”
